Inter Chicago Premier FC Registration Information
Congratulations to all new Inter Chicago Premier FC team members! Thank you all for trying out and welcome to the ICPFC family! Registration will be held at Twin Lakes Park in Wildwood on Wed and Thurs, June 15 and 16 from 5 to 7pm. Please bring the following items:

IYSA Medical Waiver
The above forms can be filled in and printed from our website under "Forms/Downloads". Please date the Medical waiver for 7/1/11. We also need you to bring with you a wallet size photo of your child, and a copy of your child's birth certificate (not needed if already a club member ). Fall fees are due at registration, 50-percent must be paid at registration. Spring fees will be due Feb. 1.
